Referral Management CentreThe Referral Management Centre (RMC) provides a single point of access for professionals to make referrals into children and young people's specialist community health services. The RMC aims to help referrals to make sure children and young people are seen by the right person, with the right skills at the right time.
Who we are
The RMC is made up of a team of skilled administrators who register and facilitate referrals for children and young people to the service(s) that will best meet the needs of the child. We work with clinical colleagues across all of the children’s services to ensure every referral receives an outcome; whether that is access to a service, signposting to another service outside of our offer or advice on how to support the child and their family.
We are located with the Child and Adolescent Mental Health Service’s (CAMHS), Community Early Intervention and Response Team and NHFT Safeguarding team which permits us to escalate high risk referrals for immediate assessment where required. We are also based with the Universal Services 0-19 Administration Hub which allows us to work freely with colleagues in their services.
In addition to this we work closely with colleagues in Northamptonshire County Council (NCC) to improve pathways for families who might not meet the threshold for specialist services but who may benefit from lower level interventions and support.
Our aim is to ensure that every referral receives an outcome.
We accept referrals from any professional who is working with a child, young person and their families. We use one generic referral form which enables professionals to refer to any service in the same way and by the same route.
We provide training and support to professionals inside and outside of the NHS in how to make referrals. We are also available via telephone to advise referrers or families on the outcome of their referrals.

Who is this service for?
Any professional who is working with a child, young person and their families and wishes to make a referral.
How to access this service
We accept referrals from any professional who is working with a child, young person and their families. We use one generic referral form which enables professionals to refer to any service in the same way and by the same route.

To refer a child to the Children's and Young People's ADHD/ ASD (Attention Deficit Hyperactivity Disorder and Autism Spectrum Disorder) the two forms below need to be completed in full.
From the 1st September 2019 we will only be accepting forms filled in via the word document online - please do not scan and upload - The referral packs should be attached to the referral form at the point of making the referral for ADHD/ASD so all of the information is screened together.

Why do we collect personal data about you?
We collect personal data from you for the purpose of providing services to you and your family. These services can be provided by a range of partner organisations which could include, but are not limited to:
• Relevant Northamptonshire Healthcare NHS Foundation Trust teams
• Relevant Northampton General Hospital teams
• Relevant Northamptonshire County Council teams
• Relevant organisation providing Emotional Well-being and Mental Health Services
• Relevant education professionals
• Relevant healthcare professionals
We will use the information to identify and make referrals to appropriate services; to monitor our work; to report on progress made; and to fulfil our statutory obligations and statutory returns as set by the law. The primary data owner will be Northamptonshire Healthcare NHS Foundation Trust (NHFT).
How NHFT protects and uses your personal data?
All personal data we collect from you is handled according to the data protection principles, as stated in the Data Protection Act 1998 (DPA). This includes collection, usage, disclosure and retention of your personal information.
Your rights.
You have the right to ask us to stop processing your personal data in relation to service. However, this may stop us delivering a service to you. Where possible, we will do as you ask, but we may need to hold or process personal data to comply with a legal requirement. If you find that the personal data that we hold is no longer accurate, you have the right to have this corrected. Please contact the service holding the personal data or the CYP Referral Management Centre.
