Adult Services - Adult Community BedsOur five community hospitals offer inpatient specialist services for adult patients who need 24-hour inpatient care. The service is for sub-acute medicine and a combination of medical, physical and neurological needs.
The patients may:
- Have had a severe illness
- Had a decline in their health after a long-term illness
- Need medical, physical or specialist rehabilitation
- Have had a stroke or a brain injury
- Our unit has dedicated beds for end-of-life care
To meet these varied needs our ward teams are made up of a range of healthcare professionals. Some are based at specific sites and others come to the ward depending on the patient's needs.

Who is this service for: Adults who need sub-acute care with ongoing treatment, physical therapy and further support.
How to access this service: You cannot self-refer to Community Hospitals. Other hospitals or specialist clinicians will refer you if they think that this is in your best interests.

Meet the team
Doctor
The doctor will review your current medical needs and set out a treatment plan. This may include referrals to other specialists, changes in your medication and blood tests. This information will be passed on to your GP.
Nurse
The nurses will provide quality care throughout your stay, specialising in rehabilitation and sub-acute care. The nurses will carry out physical observations and assessments. They will communicate with you and your family and document your care on a computerised patient system.
Physiotherapist
The physiotherapist will assess your functional ability, including your mobility, and develop a programme of rehabilitation that other staff will follow to improve your independence.
Occupational therapist
The role of the occupational therapist is varied and involves assessing your ability to cope with everyday activities such as washing, dressing and meal preparation. They will discuss your home situation and order any equipment you may need when you return home.
Therapy assistant
The therapy assistant works alongside the physiotherapists and occupational therapists to help you achieve your goals. This can involve washing and dressing practice, mobility practice and arranging group activities with other patients on the ward.
Ward clerk
The ward clerk will be at the ward reception area and will meet and greet visitors to the ward. They carry out various administrative tasks for the ward.
Ward housekeeper
The ward housekeeper will ensure your bed area is ready for your admission. They will ensure the ward is clean and tidy with the support of the domestic team. The housekeeper may help you complete your meal choices.
Discharge co-ordinator
The discharge co-ordinator will assist in making arrangements ready for your discharge. They may arrange a progress meeting with yourself and family members to discuss your progress and plan your discharge. Your progress will be discussed and what steps are needed for you to go home safely.
