Adult Services - Unplanned Intermediate Care TeamOur team aims to give rapid assessment, treatment and support to patients who have had recent decline in their physical health, which might make them at a greater risk of admission to hospital. We achieve this by supporting people in their own homes or helping them to leave hospital. We work towards maximising independence and helping patients to go back living at home. We also deliver intravenous therapy to patients within their homes. This makes sure they are able to stay out of hospital and still receive the treatment they need.
Who is this service for? Our services are for people aged 18 years and over.
How to access this service: Telephone referrals are taken from your GP, the ambulance service, residential homes and any health or social care professional. Patients and carers can self-refer if already known to unplanned intermediate care, community nursing or evening district nursing teams.
