Adult Services - Unplanned Intermediate Care Team
Our Urgent Community Response (UCR) team provides rapid assessment, treatment and support to patients who have experienced a recent deterioration in their physical health, placing them at increased risk of hospital admission. We deliver timely interventions (treatment and advice) within patients' own homes or their usual place of residence, helping to avoid a visit to or a stay in hospital.
We also support patients to leave hospital sooner by providing ongoing assessment, treatment and monitoring in the community. Working closely with multi-disciplinary colleagues, (colleagues in other teams) we focus on maximising independence, promoting recovery and enabling patients to remain safely at home.
The team manages a wide range of acute (sudden or serious) health needs, including infections, falls, frailty, flare ups of long-term conditions and other urgent healthcare concerns. In addition, we provide Outpatient Parenteral Antimicrobial Therapy (OPAT), delivering intravenous (through the vein) antibiotics and other intravenous treatments within patients' homes. This enables patients to receive the care they need in a homely environment while reducing the need for long hospital stays.
Who is this service for? The service supports adults aged 18 years and over who are registered with a Northamptonshire GP and have a primary physical health need requiring urgent assessment, treatment, monitoring, or support within the community.
How to access this service: Referrals to the Urgent Community Response (UCR) team can be made through our Single Point of Access (SPOA) via telephone. We accept referrals from GPs, ambulance services, NHS 111/999, community services, care homes, and other health and social care professionals. Patients and carers who are already known to the Urgent Community Response (UCR) or Community Nursing teams can also refer themselves directly for support.
