The Virtual Clinical Care Team is here to support with remote nursing assessments to help manage long-term conditions. We aim to provide safe, compassionate, and effective care that helps patients maintain independence and wellbeing.

After your referral

What to expect once you've been referred to our team

Once a referral is made to our service via the Single Point of Access (SPOA)

  • Our team will review the referral to ensure it meets the criteria for virtual support.
  • Once the team have confirmed you are suitable for remote monitoring, you will be visited by Assistive Technology from West Northamptonshire Council to install your kit - this will consist of 1 x tablet device, a blood pressure machine, oxygen saturation probe and thermometer. 
  • We utilise a platform called Luscii - which will be installed already on your tablet. Luscii is the digital platform where your data is collected. You may be asked to send a photo via the Luscii platform. This will also be uploaded to Systm One, so that photos can be reviewed by any other relevant practitioners such as your GP, or the Urgent Community Response team. You may also be sent messages within the app containing supportive self-care literature.
  • You are expected to submit data to the team daily . A member of our administration team will be in touch with you if you have not submitted any data within 48hrs. Patients who fail to submit data regularly (3 occasions within 3 months) will be referred to the service Matron to review for discharge from the service.
  • We will work with you to develop a personalised care plan based on your clinical needs and goals.
  • We aim to respond promptly, but please note that at busier times the response time will be delayed. It is essential that in the event of requiring urgent medical attention, such as new, sudden chest pain, new one-sided weakness, or loss of blood, you should NOT contact the Virtual Clinical Care Team, but instead call an ambulance via 999.
