The Virtual Clinical Care Team is here to support with remote nursing assessments to help manage long-term conditions. We aim to provide safe, compassionate, and effective care that helps patients maintain independence and wellbeing.

Adult Services - Virtual Clinical Care team

Our service is provided by a small team of Registered Nurses and Paramedics, providing targeted nursing interventions in your home or place of residence. 

We are supported by two GP's for support with clinical decisions. These decisions may include, but are not limited to:

  • Long-term disease management
  • Medication monitoring
  • Unwell adults assessments
  • Promoting independence and self-management of care

We offer planned nursing care between 07:30am and 23:00pm, 7 days a week. This includes weekends and bank holidays, ensuring we are available to provide care when you need it most.

We aim to provide the right care at the right time. While we always strive to respond to concerns promptly.

Please be aware that demands on the service can fluctuate, meaning we must prioritise unwell patients and delay contact times for patients who are more stable.

Referrals are accepted from GP and other healthcare professionals.
