TypePlanned Care and Recovery Treatment Service
We work in a recovery focused way promoting co-production in all our work; this means working with service users with mental illness and their carers to develop their assessments and plan of care to ensure their voice is at the centre of all that is provided.
Our team also works with service users who have more complex needs requiring a full multi disciplinary team approach and case management over a longer period of time, to develop recovery models with the aim of supporting service users to reach the optimum recovery and goals they have set for themselves. The teams work as flexibly as possible and if a service user requires additional support from the wider service to prevent a crisis escalating, then they we call upon resources across the pathway such as home treatment without the need for additional referrals.
Who is this service for? Adults aged 18 years and over with mental illness.
