Planned Care and Recovery Treatment ServiceWe work in a recovery-focused way promoting co-production in all our work; this means working with service users with mental illness and their carers to develop their assessments and plan of care to ensure their voice is at the centre of all that is provided.
Our team also works with service users who have more complex needs, needing a full multidisciplinary team approach and case management over a longer time, to develop recovery models with the aim of supporting service users to reach the best recovery and goals they have set for themselves. The teams work as flexibly as possible and if a service user requires additional support from the wider service to prevent a crisis, then we call upon resources across the pathway such as home treatment without the need for extra referrals.
