Urgent Care and Assessment TeamThe Urgent Care and Assessment Team works to ensure that a responsive assessment is provided by our multi-disciplinary team. This may result in an urgent short term work either as a home treatment, on an individual basis in Primary Care, or through psychological based group work.
The Community Mental Health Service is multi-disciplinary in nature. Within the team, there is a range of professionals with different skills and levels of training to benefit the service provision. The service provides the following commissioned functions under one umbrella:
- Crisis Resolution and Home Treatment.
- Primary Care Liaison Service.
- Crisis Telephone Support Service.
This is provided through an integrated single service so there is no requirement for referral between services, services can be increased or reduced dependent on service user and or carer need.
