The Referral Management Centre (RMC) provides a single point of access for professionals to make referrals into children and young people's specialist community health services.

Professionals working with a child, young person and their families can make a referral into specialist services.  Parents and care givers cannot make a referral. 

Referrals should be made electronically via SystmOne (GPs and other healthcare professionals) or using the online form on this website.  Referrals are not accepted via email or the post.

How to make a referral for specialist children's services using SystmOne.

We have updated the form, including

  • A new layout - making it easier to capture and process the referral
  • Save and complete later - the option to save your referral in draft, before completing it at a later date
  • Mandatory fields - allowing us to capture the information we need to process the referral first time, allowing us to provide clinical assessments for your patients quicker
  • Attachments for supporting evidence - please note, we can only accept files that are of 5MB or less to ensure SystmOne patient record compatibility

To refer a child to the Children's and Young People's ADHD/ ASD (Attention Deficit Hyperactivity Disorder and Autism Spectrum Disorder) service the parent and school pack forms need to be completed in full, including the consent page at the beginning of the document to ensure the clinical team can review the referral. 

These should be completed electronically and sent in with the referral, or as additional information if requested.

Professionals can refer children and young people to the following specialist services in Northamptonshire through the Referral Management Centre:

  • Attention Deficit and Hyperactivity Disorder - ADHD
  • Autism Spectrum Disorder - ASD
  • Bereavement Service - NGH
  • Children's Clinical Nurse Training 
  • Children's Community Nursing - NHFT and NGH
  • Children's Continence Service - 0-19 continence pathway must have been followed priori to referring
  • Community Eating Disorder Service
  • Complex Feeding Difficulties Service
  • Consultant Community Paediatric - NHFT and NGH
  • Community Team for People with Learning Disabilities (CTPLD)
  • Dietetics
  • Emotional Wellbeing and Mental Health including CAMHS and Youth Counselling
  • Healthy Lifestyle Team / Weight Management -  if a BMI is above the 98th centile, please refer to Dietetics
  • Infant Feeding Consultant / Breastfeeding
  • Looked After Children - Mental health only. Not for medicals or health assessments
  • Occupational Therapy
  • Physiotherapy - NHFT and NGH
  • SALT - Communication
  • SALT - Feeding
