Healthy communities: bringing care closer to home

Across the NHS, there is a firm focus on shifting health care and treatment away from hospitals and into communities - bringing services closer to where people live and helping to reduce the need for treatment in hospital.

We work in the hearts of our communities and our efforts are focused on working with local NHS providers, councils and other partners to build the health and wellbeing of our communities together.

We have listened to what our local communities have to say and we are committed to working together to provide care that is centred around people's needs.

Over the next five years we aim to:

  1. Empower our communities to live with more independence and avoid unnecessary hospital stays.
  2. Identify and tackle health inequalities (unfair and avoidable differences in health and access to healthcare between different groups) in our neighbourhoods and build prevention of ill health into everything we do.
  3. Be community focused using our status as an 'anchor organisation' to add social value to the communities we serve.

Anchor organisations are large organisations that have an active and significant stake in their local areas. Because of the size of our organisation, and as a major local employer, we can help to provide social and economic benefits for our communities, which in turn can support improved health and wellbeing.

Our Social Value Charter highlights our commitment to social, environmental and economic improvements across Northamptonshire. We pledge to contribute to our local communities beyond our day-to-day work to reduce avoidable inequalities and improve health and wellbeing.
