GP Partnership Celebrates 12 Months of Achievements

On Monday 7th October, the 3Sixty Care Partnership held its annual review meeting. The 3Sixty Care Partnership is a unique joint venture between a federation of 27 GP practices in Kettering, Corby, Wellingborough and East Northamptonshire and Northamptonshire Healthcare NHS Foundation Trust that aims to bring health and care professionals together to provide community based prevention programmes and joined up health and care treatment and support for our patients where they live.

It’s been an eventful year for the 3Sixty Care Partnership, which has enjoyed numerous successes including the achievement of tenders to provide health checks and GP extended  access. This year also saw the launch of a number of services to improve patient access to clinicians, including First Contact Physiotherapy, which allows patients with musculoskeletal problems to access a physiotherapy consultation without being seen by a GP first. This year has also seen further development of the Care, Assessment and Treatment of Children at Home (CATCH) service, which is now able to offer more same-day appointments to children age 5 years and under with an on the day minor  illness. Likewise, the 3Sixty medical academy has continued to develop the student placement programme and increase the number and range of opportunities on offer, which includes primary and community care placements, integrated care placements, medical apprenticeships, and physician associates.

Primary care is essential to the delivery of the NHS Long Term Plan, and 3Sixty is well placed as a dynamic and sustainable organisation to lead five of the Primary Care Networks (PCNs) in Northamptonshire, representing around 300,000 of the local population. 3Sixty is able to take responsibility for the contracting, performance management and reporting requirements, allowing practices to focus on great patient care.

Overall, it has been a great year for the 3Sixty Care Partnership and we are looking forward to continuing the progress into the next 12 months. Thanks to everyone who came along on the evening to review the year’s achievements with us.
