Community services are essential for everyone, everywhere

Community Services Week is an opportunity to recognise the vital role community healthcare services play in supporting people across Northamptonshire every day.

At NHFT, our community services help people stay healthy, maintain their independence and access the care they need in their homes, communities and local settings. From specialist nursing and therapy services to screening programmes and public health support, our teams work with people of all ages to improve health and wellbeing and deliver care closer to home.

This week, we are shining a spotlight on just a few of the many services that make a difference to local people and communities every day.

Supporting people at home through community nursing

Community nurses play an essential role in helping people receive high-quality care in their own homes and local communities. Supporting individuals with a wide range of health needs, the service provides assessment, treatment and ongoing care for people who may be living with long-term conditions, recovering from  illness or managing complex health needs.

Working closely with GPs, hospitals, social care teams and other healthcare professionals, community nurses help people remain as independent as possible while receiving the care and support they need. From wound care and medication management to supporting people with long-term conditions and end-of-life care, the team delivers compassionate, person-centred care where it is often needed most.

By bringing healthcare closer to home, community nursing helps reduce unnecessary hospital admissions, supports timely discharge from hospital and enables people to receive care in familiar surroundings, improving both their experience and outcomes.

Supporting families affected by dementia through Admiral Nursing

NHFT's Admiral Nurses provide specialist support to families living with dementia. As dementia can affect every aspect of a person's life, the service offers expert guidance, practical advice and emotional support to both individuals with dementia and the people who care for them.

The team works closely with families to help them navigate challenges such as changes in behaviour, planning for the future and accessing the right support at the right time. By providing specialist knowledge and reassurance, Admiral Nurses help families feel more confident and supported throughout their dementia journey.

Helping people move, recover and live independently through physiotherapy

Physiotherapy services support people to improve their mobility, manage pain and recover from illness, injury or surgery. Whether helping someone regain confidence after a hospital stay or supporting the management of a long-term condition, physiotherapists play an important role in helping people achieve their goals and maintain their independence.

Working across community settings, the service focuses on personalised care, empowering people with the knowledge, exercises and support they need to stay active and improve their quality of life.

Additionally, our Physiotherapy Symptom Checker, developed by the team, can help you manage your symptoms while you wait and guide you towards the right kind of care.

Promoting health and wellbeing through sexual health services

NHFT's sexual health service s provide confidential advice, testing and treatment to support the sexual health and wellbeing of people across Northamptonshire.

The service offers a wide range of support, including contraception, sexually transmitted infection testing and treatment, and information designed to help people make informed decisions about their health. By making sexual healthcare accessible and stigma-free, the team helps individuals take control of their wellbeing and access support when they need it.

Alongside clinic-based services, NHFT's Sexual Health Outreach Team works directly within local communities to improve access to sexual health support. The team engages with groups who may face barriers to accessing traditional healthcare services, delivering education, advice, testing and prevention initiatives in a range of community settings. By building relationships with local organisations and communities, the team helps ensure that people can access confidential, inclusive and non-judgemental support when and where they need it.

Preventing sight loss through diabetic eye screening

The diabetic eye screening programme helps people living with diabetes protect their sight through regular screening appointments.

Diabetes is one of the leading causes of preventable sight loss, and screening plays a crucial role in detecting changes to the retina before symptoms develop. Early identification allows treatment to be offered when needed, helping to reduce the risk of serious complications and protect people's vision.

The programme supports thousands of people across the county each year, providing an important preventative service that helps individuals manage their long-term health.

Caring for communities every day

These services are just some example s of the breadth of community healthcare provided by NHFT. Every day, colleagues across our community teams support people to stay well, access specialist care and live as independently as possible.

Community Services Week provides an opportunity to recognise the skill, compassion and dedication of these teams and to celebrate the positive impact they have on the lives of people, families and communities across Northamptonshire.
