Community nursing team making a difference

Morgan Allinson-Johnson community nurse

To celebrate the amazing work of NHFT's community nursing team, we went out and about with Morgan Allinson-Johnson as she completed her daily rounds.

Morgan has worked as a community nurse for over two years and covers a wide area, seeing patients in their own homes and in local care homes.

Our Community Nursing service provides a range of services, from post-operative dressing changes to end-of-life care and long-term condition management such as diabetes. They also provide a small number of clinics.

Community nurses provide high quality care for people in their own homes to promote and maintain independent living through information, guidance and training for patients, carers and families.

Supporting people where they live means patients are less likely to have to go to hospital, which reduces the burden on NHS hospital beds.

Our Community Nursing team is made up of a variety of skilled healthcare practitioners including:

  • healthcare assistants (HCAs)
  • registered nurses
  • district nurses
  • advanced nurse practitioners
  • phlebotomists (who collect blood samples)

Patients who are discharged from hospital, that require Community Nursing input, will be referred directly to the service by the ward.
