RECONNECT - Care after custodyReconnect is a service that supports you in taking care of your health after you leave prison and are living in England. If you will not be living in Northamptonshire, we can refer you to other Reconnect services in England. The service can support you with the transition of leaving prison and accessing community-based services for your individual health or care needs.
Our service can support you for up to 6 months after release. We can help you to start using health, care services or other support services that meet your agreed health and care needs plan.
Following referral, we will ask for your consent to do an assessment. In the assessment we will ask you some questions about yourself. This will help us find out about any physical or mental health vulnerability, substance misuse needs and any additional needs which may be a barrier to you using community-based services.
Who is this service for? Reconnect works with people over the age of 18 years with a health or care need. Referrals can be made up to 12 weeks before release and within 28 days after release. You can refer yourself to the service or a referral can be made by prison staff, other Reconnect services, probation, liaison and diversion services, or your family.
