Adult Services - Evening District Nursing TeamWe deliver both planned and unplanned nursing treatment for people in their own home which is required during the evening. We aim to prevent unnecessary admission to hospital and proactively manage health needs.
- Planned care is for adult patients who have a nursing need which for clinical reasons has to be delivered outside of the operating hours of the Community Nursing Service which finishes at 6pm daily. This is for people who have short-term needs and are housebound.
- Unplanned nursing care involves the rapid assessment, treatment and monitoring of patients who have experienced a recent trauma or deterioration in their physical health and are at risk of admission to hospital.
- We also deliver intravenous therapy with intensive monitoring to patients within the community to ensure they are able to remain out of hospital.
Who is this service for? Adults 18 years and over.
How to access this service: Telephone referrals are taken from your GP, the ambulance, residential homes and any health or social care professional. Patients and cares can self-refer if already known to unplanned intermediate care, community nursing or evening district nursing teams.
